I dont have it (any longer) but Adobe Acrobat 4.0 (not reader) will do
it if you can find the cd. It was the 'standard' for commercial print
shops back in that era and comes with a PDF printer you can install on
Windows 9X/NT.
Else you can use any postscript driver you can install and setup a fake
printer. Then when you print click save to file. Be sure to embed
fonts when you print if you can or it will substitute them, which
should be fine if its on the same system, but will do wierd stuff if
you convert on another machine unless you have the exact same fonts. To
convert PostScript file output you should get take that save file and
convert it with Ghostscript from Postscript to PDF.
Options were kind of limited back in those days so I dont know much
other substitute software. Anyone who did serious PDF work usually
just went the adobe route.
