The very early ones had serious problems with quality control, one of the
problems was poor or none existant through hole plating.

There were various updates in the ROM software, which might explain the
differences between the undamaged ROM and the HEX listing. The only way
forward is to program an EPROM as per the listing, then try it with the ROM.
If that fails program all exactly as per listing and test again.
