I hope everything will work 100%, always, and at every reactor
everywhere. But things not always go the way you want it.
Sometimes  a l l  safety devices refuse to work about which afterwards
thick reports are written about. "This only happens once
in a million years" 

Sometimes the power is gone because of a short cut. It sometimes
takes hours to find the reason. Software has error. Because
of the wrong materials the cooling fluid leaks. The sensors
which measure that, are all defect because they were not
checked. Afterwards it appeared the all the produced sensors
that month had a problem. All things that should not happen, suddenly
happen all at once.... 

Years ago a NASA employee warned the rubber sealings
of the main space shuttle rocket get too hard because
of the very low temperature of the combustion fluids.
If they are hard like rock, they do not seal properly
the tanks. He was ignored. Leaking fluid started to burn after
the rocket was launched. The crew died, big explosion.
An awful event. Things go wrong even after warning.
Happens everywhere. You eg cannot mess with fluid oxygen.

I used to work for a Swiss manufacturer of main water valves.
They decided to make plastic valves of PVC, with an
interior of stainless steel, as they
are lighter at the work site. You don't need
a dozer to lift them; can be done by hand.
They tested the models for five years in the lab
by hanging them at 2 pieces of pipe with weights
hanging at them. Then closing them, opening them, 
by moterdrives, constantly. They worked ok. 
They were introduced to the customers
and sold afterwards by the thousands. After some 
time the complaints came and broken valves came back.
The csutomers all got credit notes. When valves
and pipes are buried, afterwards often heavy trucks
or other big vehicles drive over them.(bulldozers, digging
machines, dumpers) They could not stand it....

Computers show up error, people too, materials
fail, complex systems fail, and like Andy wrote there 
are always fools around, doing things you do not expect. 
Some people in Holland tank petrol in their diesel.
The cars get on fire. A workman lights up a
cigarette in a high container with wheat. (dust)
I knew once a man who drove an old english bike:
a Norton Commander. He rode into a long curve
and heard the lock clicking under his frontlight.
He could not steer anymore but to the right.
He got an accident. He stills dreams about it.

In Holland sometimes trains ride through switches
or red lights. Big cranes fall down here.  

Also very bright people sometimes have a bad day.
