That request would be easy to do in either Windoze or Linux. In MS Word 
or MS Works, you can scan in the document and have it creat a template 
document. This could then be reduced to a ASCII document to remove all 
the header and formatting information. This can be distributed as a 
blank form that the user during the mission would then fill in the blanks.

I am thinking that once the blank form is filled out, it would be saved 
in the "pending_xmt" folder. The radio operator can then punch up the 
file and send it. once it is sent, it is moved to the "mission_sent" 
folder. Our set-up here is 2 or 3 computers. First is the server/radio 
connection. The second, or more if needed, would be the "data input" 
boxes. My equipment is already set up as an adhock file share "similar 
to peer-peer but wireless" with 2 lap-tops. Either one can function in 
either position at this point. Also have a desktop that with a little 
more effort can also be pressed into service, but only if there is 
sufficient power/room/need to warrant that. All of these work in sinc as 
a network service. It is also, if this is thought to be a small mission, 
capable of working off one computer.

My interface from PC/radio is generic, and will work on just about any 
HF radio. I do need to make an adapter that would let me run it on a VHF 
radio I have here. (To many projects and not enough 
$/Time/motivation?/.... :-)
